Job Description

Legacy Care Assisted Living          FT and  Perdiem LPN or RN  8AM to 2PM and 2PM to 8PM

Goal:  Provide, plan, coordinate and manage nursing care, services and health education to Legacy Care Assisted Living residents, while adhering to facility policies and procedures, and in compliance with federal, state and EOEA regulations.

Required Qualifications:

·       Must be a graduate of an accredited school of nursing; licensed nurse in Massachusetts; 

Must be able to fluently read, write and communicate in English

Performance Standards:

   Follow established standards of nursing practices and implement facility policies and procedures.

   Supervise and evaluate all direct resident care provided, and initiate corrective action or disciplinary procedures as needed.

   Report any resident problem, staffing issue or safety hazard promptly, execute corrective action, if able, and notify the Resident Care Director  

   Recognize and respond to emergency situations in a calm, organized and effective manner.

   Assist with resident care, ADLs, feeding, toileting and answering lights and alarms when able.

   Assume responsibility for compliance with federal, state and local regulations.

   Assist and document admissions, transfers and discharges of residents.

   Supervise distribution of nourishments, prompt response to call lights, alarms and monitors, and answer them when able.

   Ensure pharmacy delivery of medications directly to the residents’ rooms, as prescribed to them.
