Job Description
ESSENTIAL DUTIES AND RESPONSIBILITIES
The essential functions include, but are not limited to the following:
- Provide skilled nursing treatments as ordered by the client’s doctor
- Complete evaluation tasks, including vital signs and medication reviews
- Coordinate with physicians, therapists and other individuals in the client’s care plan
- Assess and chart observations of the client’s condition at each visit
- Maintain complete and accurate documentation of nursing care
- Work collaboratively with all members of the health care team
- Provide education to clients and families on proper home health care procedures and strategies
- Record patients' medical information and vital signs.
- Monitor, record, and report symptoms or changes in patients' conditions.
- Administer local, inhalation, intravenous, or other anesthetic
- Prepare patients for and assist with examinations or treatments
- Modify patient treatment plans as indicated by patients' responses and conditions.
- Order, interpret, and evaluate diagnostic tests to identify and assess patient's condition
- Monitor all aspects of patient care, including diet and physical activity
- Engage in research activities and participation in formal educational training activities related to nursing.
- Perform other duties as assigned
MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)
- Associates degree required; Graduate of an accredited state approved, Bachelor’s Degree from school of nursing preferred
- Current and valid Connecticut Registered Nurse License
- 2 years of nursing experience, at least 1 year in home health care (preferred)
- Current CPR certification (required)
- Driver’s License
- Car Insurance
- Car Registration
- Strong interpersonal skills that build personal rapport and establish trust with the client
- Computer literate
- Strong organizational skills
- Excellent written and communication skills
- Ability to meet physical demands of the job (ex: lifting 50 pounds for transfers)
- Home health care experience
- Bilingual (Spanish) preferred
