Admission Nurse Registered Nurse RN

Duties:

·         Meets with new resident and family members upon arrival for general admission orientation.

·         Completes physical assessment, uptake forms, medication reconciliation from the discharge summary, initial care plans, immunization records, and other forms relevant to the admission process.

·         Clarifies all ambiguous medication/treatment orders at the time of admission.

·         Occasional visits to the hospitals to meet with potential residents or discharge planners.

·         Communicates all follow-up appointments to Medical Records.

·         Completes admission duties and paperwork on the unit where the new resident is assigned.

·         Communicates with the wound care/infection control RN on any skin issues and vaccination status for all new admissions.

A current, unencumbered New York State license is required.

Long term care experience is preferred.
