Spring Creek Nursing and Rehab is currently seeking an Assistant Director of Nursing (ADON) (RN) for our long-term care (LTC) and skilled nursing facility. 

Scope: Assumes administrative and clinical responsibility and accountability for managing patient care on the units. Assists in planning, coordinating, organizing, directing, controlling and evaluating functions so the objectives and standards of nursing care are met on a continuing basis.

Job Responsibilities include but are not limited to the following:

*Assists the DON with accountability for the units during the shifts worked to ensure continuity of care and communication.

*Gathers data to provide efficiency and/or eliminate nonproductive activity.

* Provides direction through instruction and delegation.

*Investigates, intervenes, and provides solution to employees.

* Acts as role model for the staff, remains calm, organized and acts in a professional manner during crisis or hurried times.

*Evaluates follow through of delegated responsibilities to staff checking for completion and thoroughness.

*Provides guidance and education to staff during situational occurrences that promote quality care.

*Promotes attendance at unit or departmental staff meetings, in-service classes and at professional conferences.

*Assist with reviewing and evaluation staffing needs and matrix adjustments to meet NHPPD and particular unit needs in order to provide for safe cost effective care; and assists DON in obtaining and receiving information and justification when variances of staffing NHPPD occur

*Conducts rounds on the units providing review and discussion regarding staff concerns, recent orders, and incident risk reviews.

*Responds to cardiac/respiratory arrests as applicable, assist DON with review of documentation as needed, with focus on any needed educational components.

*Reports occurrences of concern to DON.

*Reviews records and residents’ status to assist with pharmacy review and prior authorization completion.

*Implements and ensures administration, observation, charting and reporting effects of medications and other therapeutic and diagnostic procedures as assigned, to ensure continuity of care.

*Evaluates necessary care to be provided to residents and assigns appropriate staff.

*Initiates orders as given per physician both by phone and verbally.

*Demonstrates a high degree of judgement and initiative in the care and treatment of acutely ill patients/residents.

*Reviews and initiates corrective action with review by DON following reports of medication and treatment errors.

*Coordinates the formulation of individualized plan of care to provide specific treatments for the patient/residents.

*Assists with development and documentation of short and long term goals, utilizing interventions, expected outcome, and time frames prior to team conferences.

*Assigns and evaluates resident discharge plan to ensure accurate and complete documentation is present; utilizing plan of care, nurses notes, physicians orders and team conference notes.

*Reviews and evaluates interventions and implementation of plan of care to determine effectiveness of further needs of patient/resident using past data, physician orders and current status of physical and psychological functioning.

*Participates in Nursing Collaboration to maintain awareness of change and provide input into development and initiation of nursing/facility policies. 

*Assists with overseeing orientation of new employees and appropriate staff by reviewing unit and facility policy and procedure, establishing duties and making those persons aware of their responsibility in continuity of care.

*Participates in clinical on-call rotation schedule, addressing and forwarding concerns and issues that may arise in the facility.

*Alternates into NFRN role as needed to assist with RN staffing obligation.

*Assures sufficient supplies and equipment are available on units and assists with ordering and maintaining  adequate inventory

*Assists nursing staff as needed in physician rounds to ensure compliance with regulatory standards and residents medical needs.

*Recommends employees for transfer, promotion, training and/or dismissal in conjunction with Human Resources Department.

*Evaluates employee’s performances; utilizes evaluations as an effective management tool; completes evaluations in a timely manner.

*Completes personnel reports, discusses problems and counsels as necessary.

*Monitors and documents performance of staff and utilizes appropriate disciplinary measures consistently and fairly.

*Maintains confidentiality of patient/resident, employee and facility information at all times.

*Works closely with the DON, other department managers, nursing supervisors, LPN’s, nursing administration, medical staff, therapy disciplines, patients/residents’, families and visitors.
