Job Description

 Job Summary: To work under the direction and supervision of RN/LPN staff on duty and CNA Team Leader (where applicable) while providing day-to-day direct resident care in a manner conducive to their comfort and safety and in accordance with current Federal, State, local and facility regulations, guidelines and standards.

 Duties, include but are not limited to:

  1. Follow all facility and departmental policies, guidance, and directives when performing tasks and using equipment and supplies.
  2. Cooperate and coordinate with other personnel to be sure resident care comes first.
  3. Maintain CONFIDENTIALITY of all resident care information.
  4. Honor the resident’s personal and property rights at all times. Turn in all found articles to the nurse.
  5. Assist with direct care of residents, including but not limited to: answering call lights, baths/showers, grooming, personal hygiene, dressing, making beds, meal service, promoting hydration by providing and offering fresh fluids, toileting and perineal care, monitoring vital signs, environmental issues and any other duties assigned.
  6. Providing and assisting with showers to all residents according to set schedule, preference, and/or as needed. Do not assume that if the resident refuses to be showered that they will refuse again if asked later. Keep trying throughout the day and every day following until they are showered. Residents are to be bathed completely with regard to their privacy. Report any open areas or new skin problems to the nurse immediately. If resident has a dressing that comes off during bathing, or any other time, inform the nurse immediately so the dressing may be replaced. All open areas should be treated. Failure to notify the nurse could result in disciplinary action.
  7. Continually check assigned residents for cleanliness and personal hygiene needs, including but not limited to:
    • Clean face and hands.
    • Clean teeth/dentures.
    • That clothing is clean (not soiled or stained), appropriate, properly arranged, in good repair and buttoned/zipped.
    • Bed clothing is only worn as intended and maintains the dignity of the resident.
    • Hair is clean and combed.
    • Nails are clean and trimmed.
    • Facial hair is shaved or trimmed as needed.
  1. Make beds after each use. Soiled linen and clothing should be rinsed out and placed in soiled laundry barrels (unless otherwise specified). Remove any soiled clothing left in rooms (except when family does laundry) so that the resident does not put them on again before they are cleaned. Never place soiled linens or clothing on the floor, for infection control reasons.
  2. Encourage and motivate residents to participate in ADLs, planned activities, occupational and physical therapy/rehabilitation in order to promote activity of mind and body.
  3. Direct, escort and/or transport assigned residents to and from appropriate dining

areas. Ensure resident is properly placed at table. Feed or assist any resident as needed. Offer and obtain substitutes if resident does not like what is being served. Always note poor appetites, complaints, requests, suggestions, and refusals and report them to the appropriate nurse.

  1. Preparation of residents for meals:
    • Take to toilet, offer bedpan or provide appropriate incontinence care.
    • Allow resident to wash face and hands (or do this for them).
    • Brush teeth or clean dentures (may be completed before and/or after meals per resident preference).
    • Dress resident, comb hair, see that dentures are in place, glasses are on, hearing aid is in place, etc.
    • If resident is to remain in room/bed, position up in bed or wheelchair, offer bib and clear overbed table for position of bed tray. Bed bound residents should be supervised on a one-to-one basis. At no time should the resident be left alone in their room while eating.
  2. Complete meal and supplement intake logs, as assigned.
  3. Turning and positioning must be performed at least every two (2) hours on all residents unable to reposition themselves (includes bed bound, geri-chair bound and wheelchair bound residents).
  4. Follow bowel and bladder program as established for appropriate residents. Record bowel and bladder output on assigned residents as directed by shift nurse.

All residents who require assistance with toileting needs must be asked if they need to use the toilet a minimum of every two (2) hours. Escort residents to and from bathroom, as needed and do not leave residents alone on toilet.

All incontinent residents must be checked and changed when needed at least every two (2) hours. At no time should there be more than one (1) incontinence pad under a resident, nor shall they have more than one diaper in place. Only a pillowcase should cover a wheelchair cushion, even for incontinent residents. Plastic diapers are only to be used when a resident is leaving the facility, per family request, or at the advise of the nurse or management. Plastic diapers must be gotten from the nurse.

  1. Report all information to the shift nurse, such as a change in a resident’s condition or personality, temperature elevation, any incident/accident, regardless of type, any change in skin condition, or any complaints reported by the resident and/or their family.
  2. Maintain safety for residents, employees and visitors per facility protocol. Ensure proper usage of safety devices for residents. Use good body mechanics. Wear your back brace and always use the appropriate lifting device for transfers: such as a hoyer lift or gait belt (unless contraindicated). Missing or damaged safety devices should be reported immediately so they may be replaced.
  3. Resident incidents and accidents are to be reported to the nurse immediately. Do not move the resident until a nurse can assess for injury. Supervisory staff may request a written statement regarding any observations of incident.
  4. Restraints must be applied only as directed by the nurse. Unless otherwise stated, all restraints must be removed every two (2) hours for a minimum of fifteen (15) minutes.
  5. Any personal injuries must be reported to the Director of Nursing, or designee, no matter how minor they may seem (Note: such occurrences must be reported on the shift in which they occur, If Director of Nursing not available, report to charge nurse).
  6. Any time resident abuse is suspected or observed, it must be reported to the Administrator, or designee, immediately. This includes:       resident-to-resident

staff-to-resident

visitor-to-resident

Any abusive behavior exhibited by a resident to a staff member or visitor should be reported to the nurse for appropriate follow-up.

  1. Report any injuries of unknown origin (bruises, lacerations, skin tears, etc.) to the nurse 

      immediately and document circumstances of discovery on an investigation form, to include time, 

      date and location of resident at time of observation.

  1. Tidy furniture and maintain a resident’s room in a neat, orderly, and safe condition. Drawers need to be clean and neat, with personal care items in plastic bags. Closets should be kept organized and doors kept closed. Bed cranks should be in the proper position.
  2. Observe and report needed repairs and service issues, such as broken, defective, or lost equipment and environmental issues.
  3. Clean used equipment and return to proper storage space after using (i.e., wash basins, bed pans, urinals, bed-side commodes, etc.). Clean large equipment, wheelchairs, walkers, commode chairs, etc., as assigned.
  4. Assist in escorting residents to and from scheduled appointments outside of the facility as needed.
  5. Attend all departmental meetings and/or inservices as required. Responsible for obtaining any information provided at a missed meeting/inservice.
  6. All other duties as assigned by supervisors.
