Job Summary and Qualifications

The Case Manager LCSW assists patients and families along the care continuum with social and emotional issues that may affect their medical care and well-being. Case manager is responsible for case management, healthcare system navigation, as well as education and coordination of available and relevant community resources and referrals. Serves as a CMS-designated qualified health care provider completing services, documentation, and insurance-based requirements. Works with patients, families, physicians, staff, inter-hospital disciplinary team, and external community partners to facilitate plan of care. 

The Senior Healthcare Centers offer specialized, primary medical care for patients 65 years of age and older in North Central Florida. We have four convenient locations.
Our Mission is to provide exceptional primary care to seniors. With our knowledge of the medical, functional and psychosocial aspects of geriatric care, we partner with patients, families and caregivers to improve quality of life. Our four Senior Healthcare Centers are outpatient departments of North Florida Regional Medical Center. We are recognized by The Joint Commission for upholding the highest standards of quality care.

Location: Senior Healthcare Center (Crown Pointe)

Schedule:  Full-time, Monday-Friday, Day shift (from 8 or 8:30 until 4:30 or 5, depending on the day). No weekends. Closed on major holidays.

What qualifications you will need:

• Masters degree in Social Work required
• Florida LCSW (Licensed Clinical Social Worker) required
• 2 years related experience preferably with geriatric population

SKILLS, KNOWLEDGE, AND ABILITIES:

• Demonstrated skills in organization, verbal and written communications, customer service, math, analysis, reading, following instruction, proficient in using computer application software. Excellent critical thinking and problem-solving skills. 

This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved).
