Responsibilities of Coordinator Heart Failure Program :

 

The successful candidate will assist with navigation of the heart failure patient, collaborating with case management, the nursing and medical team, and the advanced heart failure team. They will provide CHF education to patients, their caregiver, nursing staff, the community, and business partners as needed. The Heart Failure Coordinator will track data related to readmission and other heart failure metrics as directed for accreditation and research purposes. The Heart Failure Coordinator will also assist with the Ventricular Assistive Device Program (LVAD). They will work with patients, families, and staff, using existing and evolving standards related to this innovative program. Responsibilities include participating in the evaluation and screening of patients for a VAD in conjunction with the interdisciplinary team, supporting service and unit level leadership by providing clinical and resource assistance in the management of these patients, working collaboratively with the interdisciplinary team, and assisting in the development, implementation and evaluation of the clinical program structure, process and outcome standards. The Heart Failure Coordinator will share 24/7 call for the LVAD patient providing clinical support for staff and patient care management, and engages in troubleshooting when necessary.
