DSW SUPERVISOR

QUALIFICATIONS:

  • Supervisor experience required, preferably in the healthcare industry.
  • Excellent written and verbal communication skills. Ability to utilize LASRS system.
  • Excellent time management and planning skills.
  • Ability to problem solve in a professional diplomatic manner.
  • Must respect all clients, families, and staff. Must have reliable transportation, valid driver's license and safe driving record.

JOB RESPONSIBILITIES:

  • In-person visits with each DSW required to evaluate performance, client status changes, and emergency drills conducted to ensure services provided per ISP requirements.
  • The supervisor shall visit the facility/home of client on a quarterly basis, checking if service plan is still needed, if it is adequate or needs any revisions. Ensure all documents have correct signatures and dates. Conduct monthly phone calls to all clients. Set up state audits (research documents).
  • Supervisor must match DSW timesheets with the "Time and Attendance" report in LASRS to ensure services are provided in accordance with the ISP.
  • If DSW positions cannot be staffed, the DSW Supervisor must report to work.
  • DSW Supervisor responsible for answering after hour & staffing calls on rotation.
  • Set up client binders, check yearly. Attend quarterly mtgs. Usage of copier, fax, email.
  • DSW Supervisor must complete accident /incident reports within 24 hours. Report all accidents/incidents & complaints to Manager ASAP.
  • Able to read & understand client's POC, type the client's ISP & Universal POC.
  • Create schedules for DSW's, initially, annually and as needed.
  • All client & employee documents must be scanned into the data base and filed by the end of each month. Conduct weekly PA checks to ensure all clients have a valid PA.
  • Handle filing, generating reports, set up meetings, answer phones, ordering supplies.
  • Ability to conduct a LASRS Connectivity Test.
  • Retain proper documentation regarding all client and staff communication.
  • Any additional job related duties as assigned by Manager.
