Chateau Nursing & Rehabilitation Center in Willowbrook has an URGENT NEED to fill a Full-Time Restorative Nurse (LPN)  for a 150 bed SNF!

The Restorative Registered Nurse should have a minimum of (3) years of WC experience working for a nursing home

Responsibilities include:

• Development of restorative services using person-centered care
• Coordinates and supervises restorative programs within policies and procedures of the facility
• Conducts restorative assessments to determine the need for programs
• Completes MDS assessments relating to restorative
• Creates restorative plan of care
• Coordinates schedule and restorative program with therapists
• Education and communication of the restorative program with the nursing team
• Oversees and schedules restorative program based upon resident needs
• Reviews fall assessments, reports and interventions and care planning
• Reviews bowel and bladder assessments and interventions and care planning

Regulatory Requirements:

· Completion of a recognized Restorative Nursing Program.

· Current Nursing license in the state of Illinois.

· At least (3) years previous experience for Skilled Nursing Facility

· Current BCLS certification.
