Job Requirements:

• New York State HHA certificate
• Must be Authorized to work in the United States
• Recent (Within 30 Days) full physical
• PPD
• Proof of MMR (Measles, Mumps, Rubella) or Titer
• Drug screening
• 2 professional work references
• The ability to read, write and carry out patient care plan activities
• Must pass background check
