Job Description

Home care LPN

The primary purpose of the Homecare LPN role is to provide skilled nursing in the home as ordered by the physician on the plan of treatment and nursing care plan developed by the registered Nurse.  Spanish speaking a plus!

Responsibilities:

  • Prepares clinical and progress notes
  • Demonstrates knowledge of medications and their correct administration based on age of the patient and patient’s clinical position.
  • Follows the seven (7) medication rights and reduces the potential for medication errors.
  • Assists the Registered Nurse or physician in performing specialized procedures and duties.
  • Functions as a team member demonstrating collaboration with the Registered Nurse, physician and interdisciplinary team.
  • Assists the patient in learning appropriate self-care techniques. 
  • Promotes patient/family education 
  • Completes and submits necessary documentation in accordance with agency policy and applicable regulations.
  • Submits verification of completed visits in accordance with agency policy.
  • Ensures documentation is co-signed by the Registered Nurse  in accordance with state regulations

Qualifications

  • Graduate of an accredited nursing program.
  • Current state licensure as a LPN.
  • One to two years of medical/surgical nursing or long-term care experience desired. 
  • Prior home healthcare experience is preferred.
  • Valid state driver’s license and reliable automobile.
  • Proof of current automobile insurance.
  • Current health certificate/physical examination and TB testing.
  • Current CPR card.
  • Flexible, yet organized, with ability to exercise sound judgment.
  • Able to communicate effectively in English, both verbally and in writing.
  • Additional languages preferred.  Spanish speaking a plus
