We are currently seeking a Palliative Care Nurse for our Home Health department. The Home Health Palliative Care Nurse works with patients, families, and physician to determine the appropriate level and type of care needed, as well as provide home health visits and supportive care to patient and family. The Home Health Palliative Care Nurse suggests and implements interventions to assure goals of care are met.
 
The following statements describe the principal activities of this position identified and shall not be construed as a detailed exposition of all the duties that may be inherent in this position.
  • Reviews Home Health (HH) orders to determine appropriateness of services and collaborates with the patient, family and physician to suggest an alternate plan of care or additional services when appropriate;
  • Reviews data engines for potential serious illness program appropriate patients
  • Reviews patient's medical diagnosis, prognosis, medications, procedures and clinical course;
  • Maintains expert knowledge of available supports and services, based on client needs;
  • Evaluates complex HH referrals to determine whether case will be accepted;
  • Enters referrals in EMR as needed;
  • Completes an initial, comprehensive, and ongoing comprehensive assessment of patient and family/caregiver to determine needs.
  • Provides a complete physical assessment and history of current and previous illness(es);
  • Provides professional nursing care,
  • Initiates the plan of care and makes necessary revisions as patient status and needs change;
  • Uses health assessments to determine nursing needs and any follow up needed;
  • Develops a plan of care that identifies interventions which will allow patient and family/caregivers to meet their goals;
  • Administers medications and treatments as prescribed by the physician in the physician’s plan of care;
  • Provides education to the patient and family/caregiver as appropriate per assessment and plan of care;
  • Assumes responsibility to coordinating patient care for assigned caseload;
  • Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given;
  • Records pain/symptom management changes/outcomes as appropriate;
  • Documentation will be completed and returned to the office same day of assessment;
  • Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required;
  • Assists the patient and family/caregiver and other team members in providing continuity of care;
  • Participates in education programs, quality assessment activities, nurse’s meetings and other staff meetings as required;
  • Participates in Case Conference and IDG meetings as required;
  • Complies with Compliance Plan;
  • Complies with HIPAA Policies. Maintains confidentiality of information relating to the patient and family. Will discuss only those aspects necessary to the care and treatment of patient and family with those directly involved in the patient’s care;
  • As business needs may change, may perform additional responsibilities assigned by supervisor that are reasonably related to the position.
Education
• Must meet at least one of the following requirements:
• Registered nurse (RN) with current licensure in State of practice;
• RN strongly preferred.
 
Experience 
• Minimum one (1) year of experience with the direct care to the elderly, disabled or infirm required; 
• Minimum three (3) years’ experience working as an RN, preferably in Hospice, Palliative Care, or Oncology preferred; 
• Experience in care management, care coordination, discharge planning, etc. is preferred.
