What You’ll Do and Who You Are:

The Client Care Coordinator is to ensure Clients receive the highest quality care in accordance with Company standards. Within the framework of the objectives and policies established by the company, plans and coordinates care objectives. Evaluates and manages all office staff employed by the company providing care to clients assuring that care is in accordance with an established plan of care in accordance with Right at Home policies and procedures and all other laws and relevant prfessionals standards. Responsible for case management of Clients in accordance with Agency policies abd procedures and all other applicable procedures manuals, laws, and relevant professional standards. In summary, is responsible for Client care, clinical, resource management, and oversight of the service.

Essential Functions:

  •  Maintain compliance to all Agency clinical and administrative policies, procedures and regulatory requirements
  •  Ensures the quality of client care and services are in compliance with applicable State Practice acts and Agency Policies.
  •  Helps improve the knowledge base and effectiveness of agency staff by teaching and further developing staff. Provides in-service education for Personal Care Assistants and Homemaker Personnel.
  • Documentation of care/issues/ events in ClearCare and in client/caregivers files as directed by state licensure guidlines and RAH recommendations
  • Prepares abd records clinical supervisory notes and progress notes in Accordance with Agency policy as well State and federal regulations.
  • Attends and actively contributes to monthly team meetings

Must Haves:

  • Graduate of an accredited practical or vocational nursing program and licensed to practice in the state
  • 1 year of nursing experience (home health/hospice preferred)
  • Staff License Practical Nurse (LPN) must possess and maintain a current CPR certification
  • Knowledge, ability, and the necessary skill and judgment to perform all nursing procedures as outlined in the scope of practice in the State’s Nurse Practice Act and under the direction of the registered nurse and physician orders
  • Staff License Practical Nurse (LPN)  must have a valid driver’s license and use of an insured automobile for work or access to adequate transportation
  • Staff License Practical Nurse (LPN) must be able to read, write, speak and understand English as needed in the job
  • Participate in in-service education programs as mandated by State law (this requirement need not be met exclusively through Right at Home sponsored in-services programs)

WORKING ENVIRONMENT

  • Primarily works in Main office; travel is required to visit clients for evaluations
  • Shift times and days vary with client needs.
