We are seeking to hire an experienced Licensed Practical Nurse to join our ever-growing interdisciplinary team of home health professionals.

LPN Expectations:

• Provides direct patient care in their home under the supervision of a registered nurse.
• Follow the patients’ plan of care and provide needed treatments.
• Provide patient and caregiver education on disease processes and wound care.
• Provides accurate and timely documentation consistent with the plan of care
• Promotes positive patient outcomes.
• Participate in patient interdisciplinary case conference

Requirements to join the team as a LPN:

• Licensed Practical Nurse license required
• A minimum of one year home health experience.
• Valid driver's license and auto insurance
