Credential Requirements:

     Valid LPN License

     Up to Date CPR Certification

     TB Test completed with the past 12 months

     Physical completed within the past 12 months

     A valid Driver's license or state Identification

     Social Security Card or Birth Certificate

     COVID Vaccine is required
