LPN NEW RATE $30.00/HR

Sunnyside Care Center is actively seeking a Licensed Practical Nurses (LPN). All shifts available.

Sunnyside Care Center is Onondaga County’s leader in sub-acute and short term rehabilitation. Sunnyside Care Center is an 80 Bed Short Term Sub-Acute Rehabilitation and Long Term Care Facility located in East Syracuse, NY.

REQUIREMENTS:
1. LPN with current New York State license that is active and in good standing.
2. Demonstrated competence as an LPN in practice.
3. Leadership qualities a must.
4. The ability to work well with others and delegate professionally as needed.
5. Able to evaluate the health care status of a long-term care resident.
6. Dependability and Accountability.
7. Possess knowledge of medications and their administration.

REPORTS TO:
UNIT MANAGER-SUPERVISOR for each unit. Nurse will be responsible for their respective ends for meds, treatments, vital signs, and resident care as well as other unit responsibilities.

Responsibilities include, but are not limited to:
1. Chart in nurses notes:
2. Assist with care of any terminal or acutely ill resident.
3. Check for completed CNA documentation i.e.: Daily Care, I&O, BM, toileting schedules, and Nutrition/Hydration before leaving at shift end.
4. Review own MARS and TARS for missing items prior to leaving.
5. Check alarms, exits, wander guards, and 02 tanks/concentrators/humidifiers
6. Note MD orders – Both 60 day and telephone orders – 1st, 2nd, 3rd checks on ALL orders.
7. Weekly head to toe skin checks.
8. Cover unit as needed and as directed when the Unit Manager is unavailable.
9. Take an active role in day to day running of the assigned unit, including directing CNA’s.
10. Assist with transporting residents to and from the unit i.e.: morning exercise, assist with ambulating residents.
11. Assure ALL residents are cared for appropriately i.e.: proper position, appropriately dressed, oral care & nail care.
12. Make sure residents return to bed after lunch and residents who remain up are toileted and changed – make sure afternoon rounds are complete.
13. Clean and stock medication cart – clean medication room and treatment cart.
14. Re-order medications if needed in a timely manner – routine, prn, insulins, inhalers, eye drops. Date and initial all MDU, insulins, eye drops, etc.
15. MAR’S and TAR’s checks – (New Month).
16. Rounds with MD, phone calls to MD as needed.
17. Take telephone orders from MD – fax to pharmacy when applicable.
18. Update Care Plans as indicated.
19. Assist with orientation of new employees as directed by Unit Manager.
20. Relay new policies and procedures to staff.
21. Assist CNA’s as needed.
22. Attend ALL in-services – both mandatory and non-mandatory.
23. Complete A/I reports.
24. Assist the Unit Manager as needed.
25. Administer all medications as ordered.
26. Administer tube feedings as ordered.
27. Assist with admissions, discharges, transfers and deaths as directed by Unit Manager/Supervisor.
28. Notify Supervisor before administering psychoactive medication. Document behaviors, interventions and results of medication use.
