Duties and Responsibilities:
 
PROVIDES QUALITY NURSING CARE TO THE RESIDENT IN ACCORDANCE WITH THE ESTABLISHED POLICIES AND PROCEDURES:

  • Receive report at the beginning of the shift; may give report to on-coming shift 
  • Inform nursing staff of new admission arrivals and the expected time of arrival, as assigned 
  • Ensure that rooms are ready for the new admission   
  • Greet new admission upon admission 
  • Introduce yourself and explain all interventions /procedures to the resident
  • Attends shift staff meetings on a regular basis to assist in identifying and correcting concerns and to assist in the improvement of services 
  • Identify issues or potential issues in the delivery of nursing care to resident and implement corrective action immediately 
  • Make rounds of unit at beginning of shift and periodically to determine resident's needs, ascertain quality of care and to monitor environment for optimum comfort and safety of residents, staff and visitors 
  • Observe residents for changes in physical, mental and/or emotional conditions and report to the physician 
  • Documents pertinent resident data on charts, including the MDS 
  • Demonstrate nursing techniques and procedures and assist auxiliary nursing staff in rendering nursing care on unit 
  • Review the nurse's notes to ensure that they are informative and descriptive of the nursing care being provided 
  • Assist in developing a comprehensive care plan for residents 
  • Take and record vital signs and report abnormalities 
  • Administer medications per physicians orders and in compliance with medication policy and procedure; secures medication cart 
  • Pick up and transcribes physicians orders 
  • Count and record narcotics at beginning and end of shift; secures keys 
  • Fax drug orders to pharmacy immediately 
  • Assist physicians with examination and treatment of residents 
  • Recognize and report potentially dangerous situations 
  • Assist in the orientation and training of new employees, making recommendations and reports as necessary
  • Maintain confidentiality of resident information at all times 
  • Assume responsibility of Charge Nurse, as assigned 

ASSIST WITH THE PROFESSIONAL NURSE IN THE PROVISION OF NURSING CARE ON THE UNIT:

  • Observes, records and reports residents vital signs, symptoms and general condition to appropriate staff member 
  • Maintains records relating to resident care 
  • Records nursing care given, observations and residents' responses to nursing intervention on progress notes
  • Assists in updating care plans as resident condition changes or as scheduled 
  • Charts medications and treatments administered and residents' reaction to same 
  • Participates in gathering data on residents upon admission to provide information from which a nursing care plan can be developed 

NOTE: THE LPN WILL NOT PERFORM ASSESSMENTS ON RESIDENT'S AND WILL FUNCTION WITHIN THE SCOPE OF LPN PRACTICE.
 
ENSURE RESIDENTS' SAFETY IN ACCORDANCE WITH RESIDENT SAFETY PROGRAM:

  • Follow safety features of the plan of care
  • Keeps residents bedside unit and surroundings clean, safe and noise free as possible 
  • Ensure that the assigned work area (nurse's station, meditation room, etc.) are maintained in a clean, safe and sanitary manner.
  • Reports unsafe equipment and/or conditions to the nurse in charge 
  • Report malfunction of equipment and/or broken furniture immediately to Engineering Department
  • Participate in the implementation and maintenance of the Infection Control program for monitoring and/or reporting communicable and/or infectious diseases among resident and personnel 
  • Ensure that personnel follow the established procedures for the use and disposal of personal protective equipment 
  • Ensure that all residents are wearing appropriate identification bracelets 
  • Identify the resident by reading the ID band and checking the photograph in the MAR
  • Ensure medication is given to the right resident/patient by checking all resident/patients have the appropriate identification including wristbands before dispensing notification
  • When administering medication, remain with resident to ensure that he/she swallows or takes medications as instructed
  • Follows standard list of abbreviations of medical terminology that are used in providing care
  • Document in the MAR with initials, in the appropriate time and schedule
  • Clear medication chart upon completion, replenish supplies, return cart at the nursing station and ensure it's locked and no medication are accessible.
  • Assist in developing safety standards for the nursing service department as necessary.
  • Ensure that all visitors to the unit and staff are properly identified
  • Otherwise adhere to resident safety program while performing day to day responsibilities
  • Report observations that are inconsistent with resident safety program or violations of the policy to immediate supervisor and/or Chief Resident Safety Program Officer.  
  • Complete emergency equipment/supply checks each shift
  • Ensure that the assigned nursing personnel follow established infection control procedures when isolation precautions become necessary

 
PARTICIPATE IN THE IMPLEMENTATION OF FACILITY QUALITY IMPROVEMENT PROGRAM FOR THE NURSING SERVICE DEPARTMENT:

  • Participate in facility surveys (inspections) made by authorized government agencies as requested by the Administrator or Director of Nursing 
  • Assist in performing QA audits by gathering data and/or completing audit tools 

 
PARTICIPATE IN MANDATORY IN-SERVICE AND JOB TRAINING PROGRAMS:

  • Attend mandatory in-services annually 
  • Attend in-services, workshops and seminars to further professional development 

 
PARTICIPATE IN AND ADHERE TO EMPLOYEE HEALTH, SAFETY, SECURITY AND CORPORATE COMPLIANCE PROGRAMS AND ACTIVITIES:

  • Receive annual physical including appropriate screening for tuberculosis 
  • The ability to conduct all activities within the resident abuse prevention manual
  • Cooperate with investigations regarding employee health, safety, security, and corporate compliance programs 
  • Report all accident and incidents to Supervisor immediately upon occurrence 
  • Correct unsafe or hazardous condition encountered, if unable to correct, report condition to immediate Supervisor upon discovery 
  • Conduct all activities within established safety, security and infection control procedures and guidelines 
  • Conducts all activities within established corporate compliance policies and procedures 
  • Observe and respond to all standing security precautions, devices and alerts as they pertain to work and common area 
  • Respond to fire/disaster drills and emergencies 
  • Operate equipment and machinery in accordance with manufacturers' directions and facility procedures

  • Must have the ability to create and maintain a demeanor that demonstrates warmth, personal interest and positive empathy, as well as, a calm environment throughout the assigned work area.  
  • Must be able to treat all patients/residents, and/or visitors with kindness, dignity and respect
  • All staff members must have the ability to effectively and professionally cope with and manage interactions with patients/residents and their family members and/or visitors so as to ensure the best possible care, customer service and satisfaction.  
  • All staff members must possess the ability to effectively interact and conduct all interactions with clients, members of the public, vendors and consultants and/or anyone within the facility a professional manner so as to ensure the best possible customer service and satisfaction.
  • Must assist or get assistance for a patient/resident upon request or observation. 
  • Timely complete patient/resident grievance form, when requested
  • Timely report any complaints or grievances made by residents/patients and/or visitors to immediate supervisor
  • Timely report any observations or any suspicions of dissatisfied resident/patients and /or visitors to immediate supervisor

PROFESSIONAL CONDUCT:

  • The ability to comply with departmental dress codes 
  • Must prominently wear one's ID card and provide one's name upon request
  • The ability to maintain normal, reasonable levels of personal hygiene and professional appearance 
  • The ability to uphold the highest standards of ethics, integrity and honesty in matters of conduct and behavior as it pertains to work performance, position and profession 
  • The ability to maintain confidentiality in all matters concerning residents/patients as well as assigned computer/software and work product

  • Must possess the ability to consistently come to work regularly

  • Must possess the ability to adhere to meal/break policy/schedules
  • Responsiveness to being called/paged while on duty
  • Willingness to work overtime when requested

 
REQUIREMENTS:

  • Must be a graduate of an accredited nursing program 
  • Current registration and license to practice as an LPN in the State of New York
  • Must be CPR/BLS certified 
  • Excellent time management and organization skills 
  • Familiar with PRI/MDS documentation 
  • Computer literate preferred
  • Two years of clinical nursing experience
  • One year experience in geriatrics and sub-acute population preferred 
  • The ability to read, write, speak and understand the English language required, knowledge of other language helpful 
  • Has effective oral and written communication skills 
  • The ability to deal effectively and professionally with emotional resident, visitors or family members 
  • The ability to relate to geriatric, infirmed and sub-acute population 

 
SETTING:

  • May perform duties throughout the facility
  • May be necessary to assist in the evacuation of residents during an emergency situation 
  • Must possess sight and hearing senses or use prosthetics that will enable these senses to function so that the requirements of this position can be fully met           
  • The ability to operate a computer or use prosthetics or devices that will enable such use 
  • Is subject to frequent interruptions
  • Is involved with resident/patients, personnel, visitors, government agencies/personnel etc. under all conditions and circumstances
  • Is subject to hostile and emotionally upset resident/patients, family members, personnel and visitors
  • Works beyond normal working hours and on weekends and holidays when necessary
  • Is subject to stay or call back during emergency condition (such as but not limited to severe weather, utilities failure, evacuation, disasters etc.)
