Job Description

An established critical access hospital located in the Upper Connecticut Valley region is now looking for an experienced Licensed Clinical Social Worker to join their award-winning healthcare team.

For more than a century, this 35-bed critical access hospital has provided accessible, affordable, and high-quality healthcare. This location represents a number of services including a level IV Trauma Center, inpatient services, orthopedics and surgical services, imaging and laboratory testing, behavioral health, and rehabilitative services. 

The Licensed Clinical Social Worker will work within an outpatient clinic setting and work collaboratively with medical providers and other healthcare leaders that provide primary care services. The Licensed Clinical Social Worker will provide emotional support, mental health evaluations, therapy, and case management services to the clinic patients. There are various shift options available.

Requirements Include: 

• Licensed as a Clinical Social Worker in the state of New Hampshire 
• At least two years of working experience as a Social Worker
• Background in a primary care setting or outpatient clinic preferred
