Hanover Terrace Health & Rehabilitation Center is looking for compassionate and motivated RN or LPN to join our Team!! 

We are looking for a nurse who want to join a team committed to delivering the highest quality of care possible while facilitating a home like environment to the residents/patients in our care.  Our facility offers a variety of services to our customers.   Short-term rehabilitation on a post acute unit, long term custodial care, as well as memory care services.  

General Duties:

• Prepare and Administer medications ordered by MD 
• Assess/Evaluate resident/patient and document/report accordingly
• Assist in developing and implementing comprehensive plan of care to facilitate the highest level of functional and emotional well-being for the customer
• Perform wound care as ordered
• Communicate and Collaborate with patients and family members 
• Thorough and comprehensive documentation in electronic medical record

Requirements: 

• Graduate of an accredited nursing program
• Current NH state RN or LPN licensure
• Desire to join a caring team and be part of our community
• CPR certification preferred
• Excellent interpersonal, communication and organizational skills
• Fully Vaccinated
