$30-$38 per hour based on experience!!

Heart to Heart Home Care is seeking professional, compassionate Licensed Practical Nurses (LPN) who are enthusiastic about making a positive difference in the lives of our patients.

Licensed Practical Nurse Job Description:

The Licensed Practical Nurse is hands-on and assists the patient in their homes with medical needs, personal hygiene, medication administration, daily household chores and supports them in the community. The Licensed Practial Nurse  provides nursing services to patients under the direction of a Registered Nurse.

Licensed Practical Nurse Job Duties Include:

• Identifies and reports to the coordinating nurse the health needs of patient through the observation of their physical, social and emotional needs.
• Administers nursing care to patients according to the care plan.
• Records and reports observations and nursing care performed.
• Performs other related functions as requested by the supervising nurse within the scope of practice and related experience.
• Administering and supervising medication.
• Maintaining proper documentation.

Licensed Pracatical Nurse Minimum Requirments:

• Complete Physical with in the year
• 1st step and 2nd step PPD (if PPD is positive, we require a chest x-ray report))
• LPN License
• Driver’s license & Social Security Card
• CPR/ First Aid
• 2 References
