Full Time or Part Time.  Night shifts available - 6:30PM - 7:00AM

SUMMARY: Performs tasks and responsibilities within the framework of case finding, health teaching, health counseling and provision of supportive and restorative care in keeping with nursing care plan under the direction of a registered professional nurse.  Does related work as required.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Assists Health Care Team with recording and interpreting resident responses to their plan of care;  Maintains open lines of communication with all staff, residents and families.
• Guides, supports and monitors allied nursing personnel in care given to residents; Assists with orientation of new personnel.
• Assists Health Care Team with the collection of data that can be used to complete the Patient Assessment Instrument, (MDS+) Resident Assessment Protocol Summary; Participates in the collecting of data for the Patient Review Instrument (PRI).
• Participates in solving problems and making improvements on the units in collaboration with Nursing Management.
• Promotes safety standards throughout the facility (i.e. fire and safety); Participates in in-service education programs, attends all mandated annual programs.
• Carries out all duties in an ethical and honest manner.
• Reports willful or inadvertent potentially fraudulent activity to management or the Corporate Compliance Officer.

SUPERVISORY RESPONSIBILITIES: This job has no supervisory responsibilities.

QUALIFICATIONS - 

EDUCATION and/or EXPERIENCE: High School diploma or General Education diploma (GED).  Graduate of a Licensed Practical nursing program approved by the Commissioner of Education.

CERTIFICATES, LICENSES, REGISTRATIONS: Licensed and currently registered as a practical nurse in New York State.
