JOB SUMMARY: In accordance with facility policies and procedures, provides clinical evaluation of the direct care to residents that is of high quality and under the supervision of the Nurse Manager and DON. Receives input from GNAs and communicates timely with the RN Staff Nurse to meet resident care needs.
DUTIES AND RESPONSIBILITIES:
- Follows the facility's policies and procedures related to the role of the LPN. Follows the facility's policies and procedures that relate to all employees.
- Addresses resident care needs within the LPN scope of practice. As needed, seeks assistance from the resident's physician, Nurse Manager, ADON or other RN member of the Nursing leadership team.
- Within the scope of the LPN practice, completes ordered resident treatments timely and accurately.
- Within the scope of LPN practice, administers ordered resident medications timely and accurately.
- Fulfills the role of charge nurse when assigned (see list of charge nurse duties).
- Addresses resident and/or family concerns and involves the Nurse Manager, ADON or members of the leadership team and needed.
- Works collaboratively with co-workers and members of all departments caring for the residents.
- Completes assignments timely, thoroughly, and accurately with sensitivity and caring, Seeks assistance when unable to complete work.
- Completes or assures completion of required reporting.
- Documents care per policy requirements.
- Follows the plan of care as developed by the RN staff nurse, MDS Coordinator(s), or RN members of the Nursing leadership team.
- Maintains confidentiality of residents and facility information.
- Observes safety policies and reports any safety concerns.
- Demonstrates respect of resident rights and dignity.
- Attends all required educational activities and meetings.
- Observes all required infection prevention and control policies and procedures.
- Participates fully in the Quality Assurance and Performance Improvement activities of the organization.
QUALIFICATIONS:
- Graduate from an accredited school of nursing, current licensure with State Board of Nursing Examiners.
- One year of geriatric nursing experience preferred.
- Must be able to speak, read, write the English language.
