Job Description

Gilman Healthcare Center is a 99-bed skilled nursing and rehabilitation facility located at the crossroads of Gilman just off Interstate 57. Since opening its doors in 1969, the facility has grown from a 49-bed facility to a 99-bed, 100% Medicare/Medicaid Certified facility. For over 40 years, Gilman Healthcare Center has cared for our community members who need 24-hour skilled nursing care and rehabilitation, returning loved ones to their homes. We are committed to achieving excellence in both nursing and rehabilitative services. Our interdisciplinary approach focuses on functional outcome and successful discharges to the communities of Iroquois, Champaign, Ford, Vermilion, and Kankakee counties.

We are currently looking for Licensed Practical Nurses (LPN) that will serve an integral role in our team. This position allows our Licensed Practical Nurses (LPN) to provide direct care and support, servicing a geriatric population.

Job Description


·        To provide planning and delivery of direct and indirect patient care through the nursing process of assessment, planning, prevention, and evaluation.

·        Direct the day-to-day activities of the nursing assistants

·        Prepare and administer medications as ordered by physician and perform other drug administration.

·        Communicate changes in patient clinical condition with physician, nursing supervisor and family

·        Complete required record keeping upon residents’ admission, discharge, transfer; chart and document per facility policy

·        Committed to interacting professionally and with care toward residents, families, and other staff

 Requirements


·        Graduate or Future Graduate from an accredited nursing program

·        Hold a Current State License as a Licensed Practical Nurse

·        Able to communicate well in both verbal and written forms

·        Have a minimum of 1 year of experience as an LPN in a long-term care setting (preferred)

·        A natural problem solver

·        Are committed to interacting professionally and with care toward residents, families, and other staff
