River Haven Nursing is looking for a full time MDS Coordinator to join our team. Sign On Bonus Included!

Responsible for the timely and accurate completion of the MDS process from admission to discharge. May coordinate information systems operations for the clinical department.

This position will require that you work shifts as a Charge Nurse when needed.   

Requirements: LPN licensure within the State plus a minimum of two years MDS experience in Long Term Care. LVN or LPN may be considered with exceptional qualifications; or equivalent combination of education and experience. Must have current AANAC (RAC-CT) certification or obtain it within 90 days of hire.
