$5,000 Sign-On Bonus

This position will require 2 days at our Shelbyville location, 2 days at our Murfreesboro location, and 1 home office day

Day in the Life / Expectations

  • Process and monitor all patient assessments and ensure compliance with state and federal regulations.
  • Coordinate resident assessment and care plan meetings with the healthcare teams.
  • Collaborate with outside members of the care plan team, including resident representatives and family members on care plan meetings.

  • Valid state license(s)
  • Must be a self-starter able to work independently and efficiently
  • Reliable transportation
