WHO WE ARE: Riverchase Health & Rehabilitation Center - a 120 bed Skilled Nursing Facility located in Quincy, FL is currently seeking an RN MDS Nurse.  We are a growing facility in need of an energetic and talented new clinical professional to join our team.

As an MDS Nurse within a Skilled Nursing Facility you will be responsible for the organization and recording of relevant MDS information, the coordination of resident assessments, and overall support of the clinical staff.  Here @ Riverchase Health & Rehab, we believe in a strong TEAM approach that truly improves the lives of all our residents.  

MDS Nurse Job Summary & Qualifications:

• Participate in PPS meetings, weekly Medicare meetings, and month end meetings
• Assist in completing the Resident Assessment Instrument with the Interdisciplinary Team.
• Ensure Care Plans contain problem, goals, and interventions.
• Active RN License Highly Preferred (may also take LPN if the below requirements are met in full)
• Minimum 1-3 Years MDS Experience in LTAC, SNF, or Hospital setting
• Advanced Organizational Skills
• Knowledgeable of nursing and medical practices that pertain to LTC
• Self-motivated to learn and master the RAI Process
