At Landmark, our interdisciplinary teams collaboratively manage our complex patient panels. These teams are led by Physicians, Nurse Practitioners, and Physician Assistants, with supporting care provided by RN Nurse Care Managers, Social Workers, Pharmacists, Behavioral Health, and other employed team members.

OBJECTIVE
The Manager, Social Work is the social work leader for specific Landmark markets with in an assigned region and division, partnering closely with local Market Leadership team(s), to formulate strategies and generate outcomes. The Manager, Social Work reports directly to a Market Health Services Director, but works closely with all Market Health Services Director in a region / division based on assigned team. The Manager, Social Work will also partner closely with the Social Care Leadership Team to ensure alignment with the national vision and strategy for social work interventions and care management activities


• Plan, organize, and direct the work of professionally trained social workers who are responsible for assessing the psychosocial needs of members and providing care management to address issues related to palliative care, behavioral health and social determinants of health
• Improve and refine workflows regarding social work referral processes specific to meet the need of market(s)
• Monitor social work performance metrics to identify opportunities for process improvements and / or staff education to produce optimal outcomes
• Provide individual supervision to assigned social work staff across multiple markets, regions and / or health plan partners
• Enforce and ensure standardization of social work workflows through routine chart audits and oversight of the delivery of social work care management interventions to include feedback on objective measures but also the clinical delivery of social work services
• Respond to requests for consultation to assigned social workers as needed
• Analyze and understand reports; identify reasons for performance gaps and identify opportunities for improved performance
• Act as a subject matter expert for the delivery of social work care management interventions
• Participate as an active member of the market Quality Council, provide recommendations and updates to improve and resolve quality of care issues
• Participate as an active member of the local interdisciplinary care team (IDT) by joining huddles and participating and leading clinical case discussion
• Participate as an active member during interdisciplinary care team (IDT) meetings with the Health Plan and other community provider networks (i.e. IPA, PCP Group, Hospital Group)
• Ensure process for identification and updates to robust inventory of resources for assigned market, region / division to be accessible to all team members
• Participate in interviewing, hiring, evaluating and disciplining of social work staff
• Assist with education, in-service training and social work orientation
• Participate in resolution of conflicts between social workers and among social workers, patients and Landmark Health personnel
• Participate in all leadership meetings as directed by the market, region or divisional leadership team
• Prepare for and lead regular meetings with social work team
• Participate in targeted ride alongs with assigned social workers as well as targeted comprehensive chart reviews in order to audit and coach team members
• Act as the subject matter expert for the market / region for all social work initiatives as directed by the Social Care Leadership Team, including but not limited to palliative care and behavioral health
• Fulfill key role of improving collaboration across team members including those that provide non-clinical patient facing interventions that support the social work team
• Provide social work coverage based on market needs
• When appropriate, provide clinical supervision to eligible social workers
• Travel to assigned markets / regions to join social workers for visits and / or meetings
• Perform related duties as necessary and other duties as assigned.

• Minimum of 5 years of social work experience (internship not included) that includes direct case management and/or clinical work with patients
• Master’s Degree (MSW) in Social Work
• Applicable licensure based on assigned State
• Experience working in or with healthcare, palliative care and / or behavioral health
• Excellent communication skills and knowledge of accessing community resources
• Two years of social work supervisory experience required
• Experience in delivering home-care or field case management is preferred
• Two years as a Licensed Clinical Social Worker (LCSW) preferred
• Possession of a valid state driver’s license and access to an automobile
