Gurwin Jewish Nursing & Rehabilitation Center is a renowned healthcare provider located on Long Island, NY which offers a full continuum of services. The Nursing & Rehabilitation Center is a 460-bed facility providing services for short-term rehabilitation and long-term skilled nursing care, home care programs, an adult day health program, ventilator dependent care, palliative and hospice care, a Memory Care Unit and on-site dialysis. The Gurwin Family of Services also includes the 201-apartment Gurwin Jewish ~ Fay J. Lindner assisted living community and the Fountaingate Gardens independent living community on the Commack campus.

We are currently seeking a per diem Nursing Care Coordinator (NCC) for evening shift who is able to cover every other weekend (Sat. and Sun.) who will embody the passion and commitment required to deliver the ultimate care for our residents. 

Responsibilities include, but are not limited to:

• The Nursing Care Coordinator is responsible for coordinating direct nursing services to patients in collaboration with unit staff and also for coordinating nursing personnel services.
• The NCC in this role functions in both a managerial and clinical role and assures adherence to and support of institutional philosophy and policy in nursing practice. Through collaboration with other disciplines and with nursing colleagues on other shifts, the Nursing Care Coordinator promotes continuity of approach in both the managerial and clinical areas.
• Candidate will assume administrative duties, maintain levels of nursing staffing and be responsible for patient care requirements through-out the facility.
• Occasional coverage for the night shift is required.

Requirements include:

• Graduate of an accredited School of Nursing
• Must be licensed to practice as a Registered Nurse in New York
• Progressive experience in Nursing Administration
• Strong leadership and administrative ability
• Bachelor’s degree in nursing required
