Job Description
I. JOB SUMMARY/RESPONSIBILITIES:
• Supports the patient and staff in the coordination of patient care and internal and external resources to ensure that care is provided in a supportive, seamless, and superior customer approach at all times. Performs activities off-site, in the community, and makes patient home visits as required.
• Performs duties and responsibilities in accordance with policies and procedures and consistent with the goals and objective of the department and North Hawaii Community Hospital.
II. TYPICAL PHYSICAL DEMANDS:
• Essential: stooping/bending, finger dexterity, seeing, hearing, speaking; lifting usual weight of 10 pounds up to 25 pounds.
• Frequent: standing, walking, reaching at and below shoulder level.
• Occasional: sitting, kneeling, climbing stairs, walking on uneven ground, squatting, pushing/pulling usual weight of 10 pounds up to 25 pounds of force; carrying usual weight of 5 pounds; reaching above shoulder level.
• Operates computer, calculator, copier, facsimile, and telephone.
III. TYPICAL WORKING CONDITIONS:
• Not substantially subjected to adverse environmental conditions.
• Must be willing to work both at North Hawaii Community Hospital and in the community. Work involves extensive off-site activities, including home visits to patients.
• Must have reliable transportation in order to make visits to sites where discharged patients reside.
• May be exposed to negative responses from patient or family.
IV. MINIMUM QUALIFICATIONS:
A. EDUCATION/CERTIFICATION AND LICENSURE:
• High School diploma or equivalent.
• Medical Assistant, Community Health Worker or Patient Navigation certification preferred.
• Valid U.S. driver's license required in accordance with Hawaii state law rules and regulations.
• Driver's abstract or evidence of good driving record.
B. EXPERIENCE:
• One (1) year experience general office or clearical experience, preferably in a healthcare related setting, such as clinic, hospital, health care agency, medical insurance, physician's office, etc.
• Experience to demonstrate familiarity with human services resources in the community.
• Previous experience working with the Native Hawaiian community.
• Previous experience in working with individuals with behavioral health issues preferred.
• Prior experience working with the underserved preferred.
