Per Diem Rotating, On-Call Weekend RN Care Manager (Rockland)

Description: The RN Care Manager will be expected to handle off hour "on call" duties as assigned on a rotating basis. 5pm on Friday's through 9am on Monday's. This position oversees the quality of care delivery for Right at Home. He/She is directly responsible for the initial client assessment, client on-boarding, plan of care, caregiver training, caregiver supervision, and client counseling.

****Weekend call averages approximately up to 5 calls and an occasional home visit or intake.

Education/Experience/ Work Requirements:
• College Degree in Nursing
• Current Massachusetts RN license
• 2 years Med-Surg clinical experience
• 1-2 Years Home Health Care experience preferred
• Excellent verbal and written communication skills
• Self- directed and able to work with minimal supervision
• Standard MS Office skills
• Valid MA Driver's License
• Automobile - in good working order
***Ability to comply with Mass. Covid Mandate

Duties:
• Oversee caregivers for quality assurance and to ensure plan of care is being followed.
• Train caregivers as needed in the on Right at Home systems and processes for quality client care.
• Fill med organizers and counsel clients and families on proper medication management.
• Meet with prospective clients and their families to do an assessment and propose a plan of care.
• Act as a patient advocate for selected cases.
• Complete required Caregiver Supervisory visits, Intros/Assessments as assigned. (RNs Only)
• Communicates with health-related person to coordinate the client care plan.
• Help identify new sources of referrals and leads.
