High Lakes Health Care, part of Praxis Health, is looking for a professional Registered Nurse (RN) Care Manager to join our team!

 

An RN Care Manager at Praxis Health is a healthcare provider who assesses their patients' needs, and creates personalized care plans. The role of our RN Care Managers serves as the main point of contact between our patients and our clinic, and our RNs are critical to the success of team based care. As an RN Care Manager, you would be involved in coordinating patient's medical treatment, including administering assessments, developing care plans, monitoring medication compliance, providing patient education, motivational interviewing, relationship building with patients, serving as an advocate and champion of our patient's health, and connecting patients with social and community support systems for continuation of care.

Essential Duties and Responsibilities:

  • Works under clinical direction of Program Director and physician medical directors.
  • Provides effective care management across the care continuum, including coordination involving internal and external providers of care, families and other entities, as appropriate.
  • Identifies appropriate cost effective care alternatives.
  • Assists patients and appropriate staff, with real time decision-making in support of the coordinated care model.
  • Care Management activities include early identification of high-risk patients, concerted planning involving physicians and mid-level practitioners, monitoring of services provided by outside physicians, home health, DME, and other providers; monitoring emergency department use as well as identifying opportunities to decrease emergency department utilization and other elements of care utilization.
  • Works with pertinent internal staff to ensure all requirements related to guidelines for care management, including related policies and procedures are met.
  • Complies with corporate compliance program and policy.
  • Ensures confidentiality of patient information following HIPAA guidelines and company policies.

  • Reliable attendance and presence at work.

  • High school diploma or general education degree (GED).
  • Registered Nurse License
  • 2+ years work experience in healthcare
  • Outstanding communication skills, both written and verbal.
  • Excellent organizational skills
  • Proficient with healthcare electronic medical record, Microsoft Outlook, Microsoft Word, and Excel.

  • While performing the duties of this Job, the employee is regularly required to stand; walk; use hands to finger, handle, or feel; reach with hands and arms and talk or hear.
  • The employee is occasionally required to sit and stoop, kneel, crouch.
  • The employee must regularly lift and /or move up to 10 pounds, frequently lift and/or move up to 25 pounds and occasionally lift and/or move up to 50 pounds.
  • Must be able to sit, stand, kneel, crawl, bend and twist as needed.  Requires walking on non-level surfaces, inside and outside the office, throughout the day. 
  • Work Environment: Combination of medical office and exam/procedure room settings.
