Title: RN Case Manager
General job description
Job responsibilities: Managing the nursing care of residents and staff performance. Responsible for carrying out the established policies and procedures of the Care Center, including overseeing resident’s plan of care is carried out. Assist the Director of Nursing in various duties as assigned. Communicate with Director of Nursing.
Job qualifications:
Education and Experience-
1. Graduate of an accredited program in Registered Nurse education required.
2. Current active R.N. licensure in the state of Minnesota required.
3. Clinical experience with geriatrics, long-term care and/or rehabilitation, care plan development desired.
4. Management experience with strategic planning, service delivery and evaluation, staff performance supervision, creative problem solving and quality improvement desired.
Professional-
1. Demonstrates a broad and current knowledge of nursing including gerontological
nursing and related aspects, such as rehabilitation and mental health.
2. Knowledge of and skilled in physical assessment techniques.
3. Knowledge of and skilled in use of the nursing process.
4. Skilled in oral and written communication-communicates clear expectations.
5. Demonstrates leadership/supervision ability-motivates staff for continued improved performance, customer service and quality. Provides staff with negative and positive performance feedback.
6. Skilled in balancing creative and realistic problem-solving-proactive in anticipating and preventing problems.
7. Knowledge of Quality Improvement-welcomes change as a means to improve.

Physical-
1. Must have no physical or mental condition which would preclude the
performance of the job description.
2. Requires functional hearing ability, conversational level, to understand
residents/staff and make auditory physical assessments such as chest/heart/ bowel sounds, BP etc.
3. Requires functional visual ability to read medical records, governmental forms,
and make observation assessments of resident, staff, and environment.
4. Physical requirement of the job description:
a. In a 8 hour day, employee must be able to sit 6 hours, stand or walk 2 hours.
b. Frequent bending, stooping, balancing, pushing and pulling; occasional squatting, reaching above shoulder level, crouching, and kneeling.
c. Frequent carrying of up to 10 pounds and occasional carrying of 11-24 pounds. Emergency situations could require carrying greater amounts.
d. Frequent lifting of up to 10 pounds. Emergency situations could require lifting greater amounts.
e. Good finger and hand dexterity to handle and manipulate instruments, equipment and records.
Job duties:
Plans, organizes, directs, evaluates and implements the nursing care of residents of the primary care group for all shifts to maintain a quality of service that will fulfill the mission and be in accord with the policies of CVCC; professional and governmental standards and regulations.
1. Completes MDS and CAA for comprehensive resident assessment.
2. Develops and maintains the nursing portion of the resident plan of care utilizing the nursing process and through collaboration with residents, families, nursing team members and other disciplines. Attends the interdisciplinary care plan meetings.
3. Maintains safe and quality methods for drug distribution.
4. Maintains accurate and complete resident care records related to resident status and care provided with appropriate documentation for MDS/Case Mix, Medicare and regulations.
5. Observes nursing cares and visits with residents and family regularly to insure quality nursing care is carried out according to the plan of care and resident/ family choices.
6. Identifies and resolves care related problems or concerns of residents, family or staff.
7. Assists with the laboratory service program.
8. Participates in the Quality Improvement Program-ongoing evaluation of service delivery with participation of primary care team members.
9. Communicates pertinent information to the appropriate persons or departments (resident, family, physician, administration, other staff, outside agencies).
10. Maintains and utilizes concepts of health maintenance, rehabilitation, resident dignity and rights, privacy and confidentiality, primary holistic care and customer service.
11. Responsible that the attending physician, family and other appropriate persons are notified when a resident is ill, injured or a significant change in condition occurs.
12. Coordinates chemical and physical restraint reduction program.
13. Physician rounds and follow-up with orders.
14. Admission and discharge assessment, planning and procedures.
Provides staff leadership and supervision.
1. Assumes responsibility for and supervises nurses, TMAs, and NARs procedures
and nursing care.
2. Delegates nursing care of residents or other duties to appropriate nursing team members.
3. Provides input regarding staff assignments utilizing knowledge of the capabilities, performances and personalities of nursing team members.
4. Assists nursing team members to learn new nursing care, policies, procedures, and concepts of care.
5. Maintains and interprets CVCC and nursing department’s mission, policies and standards to nursing team members.
6. Identifies nursing team member’s work performance problems and provides feed back, expectations and/or disciplinary measures. Maintains accurate and confidential records of employee and management’s actions.
7. Motivates staff for continued improved performance, customer service and quality. Provides staff with negative and positive performance feedback in a timely manner.
8. Assists with nurse performance evaluations, completes Nursing Assistant performance evaluations or assist Staff Development Coordinator with them.
9. Communicates education and orientation needs of nursing team members to the Director of Nursing or Inservice Coordinator.
10. Assists new nursing team members to feel welcome and receive the appropriate content and amount of orientation.
11. Acts as a role model for expected quality care, attitude, and behavior.
Monitors and maintains a safe and pleasant environment for residents and nursing team members.
1. Controls the environment to provide comfort and safety to residents and staff
according to regulation and standards.
2. Promotes resident and staff safety with proper infection control measures,
equipment use, body mechanics, safety devices, education of emergency procedures and use of the resident plan of care.
3. Evaluates and investigates incidents or injuries to prevent future similar
occurrences.
Maintains knowledge and demonstrates compliance of CVCC policy and procedure,
State and Federal regulations related to long-term care, and clinical and administrative nursing skills.
Collaborates and works with the Director of Nursing, other Case Managers,
Department Heads, Consultants and Medical Director to develop programs, upgrade policy and procedure, evaluate and improve service delivery and quality.
1. Evaluates and recognizes actual or potential problems in service delivery, staff
performance, and quality care.
2. Communicates concerns and recommendations for quality improvements to the
Director of Nursing and QI program.
3. Assists in planning, implementing, and supporting new policy, procedures, and
programs for meeting regulations and improving care.
4. Demonstrates and verbalizes support of Director of Nursing, other Case
Managers, and Administration to the public and other staff. Demonstrates respect for differences in management decisions, philosophies and style.
5. Expresses honest concerns or differences of opinion in private to Director of
Nursing, other Case Managers or Administration.
Documents objective, accurate and complete resident care records, employee
performance records and administrative management records. Maintains confidentiality of records.
Other duties as directed by the Director of Nursing.
Job Relationships: Supervised by the Director of Nursing. Supervises all nursing staff working in the primary care group. In absence of other administrative staff assumes the position of authority and responsibility of the entire facility.
