POSITION DESCRIPTION: Coordinates the overall interdisciplinary plan of care for a patient from admission to discharge, in a home care environment. Responsible for furnishing services in accordance with agency policy and procedures. Acts as the liaison between patient/family and home-care personnel to ensure necessary care is provided promptly and effectively.

  • Qualifications:
  • Has RN licensure to practice in the State of Iowa
  • Must have a pleasant disposition, a cheerful attitude, and a sincere desire to participate in aiding and supporting the needs of our patients.
  • Must have patience, compassion and understanding for all patients.
  • Must be currently CPR certified
  • Must have valid driver’s license, insurance and reliable transportation.
  • Must have good personal hygiene
  • Must be able to use professional judgement
  • Responsibilities:
  • Provide care to patients in accordance with a medically approved Plan of Care
  • Perform supervisory visits for home care aide services and LPN visits as necessary
  • Skilled nursing tasks including but not limited to, lab and specimen collection, head to toe assessment, patient/caregiver education and training, port access, start IV’s, wound care and assessment, PICC cares, infusion, and G-tube cares.
  • Case coordination of new clients with additional therapies (i.e PT, OT, ST, MSW)
  • Responsible for maintaining and safeguarding the medical records of our clients and maintaining confidentiality of patients’ rights
  • Other Duties Include:
  • Planning and completing duties with minimal direction.
  • Using tactful, diplomatic communication techniques in all situations.
  • Following up with appropriate staff, residents, or other individuals regarding reported complaints, problems, or concerns.
  • Presenting a positive image for Home Care by presenting a professional, sincere and kind attitude towards guests, clients and other visitors.
  • Maintaining all certifications required for employment
  • Attending and participating in all staff meetings.
  • Physical Demands:
  • Must be able to lift/push up to 50 pounds.
  • Must be able to walk up to 2+ miles daily.
  • Must be able to lift, carry, stand, bend, squat, crawl and reach
  • Must possess sight/hearing senses or use prosthetic that will enable these senses to function adequately so that the requirements of this position and the needs of our clients can be fully met.
  • Must be in good general health and demonstrate emotional stability in order to cope with the physical, mental, and emotional stress of the position.
