POSITION SUMMARY: 

Our Medical Model Adult Day Health Care is seeking a Clinical Director to join our two communities and direct a person-centered approach in providing medical services to our community registrants. 

• Responsible for compliance with all provisions of the State Code Subchapter A 
• Responsible in coordination with Marketing Manager to act as a liaison between the facility, the community and NH departments 
• Responsible to ensure the delivery of consistent, effective and compliant services to Registrants, families, caregivers and referral sources. 
• Responsible for overseeing and ensure that registrants are provided with a Person-Centered Care within the facility.  
• Responsible for Clinical Compliance, Assignments and Scheduling. 
• Responsible to coordinate services for Registrants with services provided by community or other agency programs, including but not limited to certified home health agencies, social services agencies, clinics and hospital outpatient departments and services. 
• Assigning adequate personnel to be on duty at all times when the program is in operation to ensure safe care of the Registrants. 
• Assigning and supervising activities of all personnel to ensure that Registrants receive assistance in accordance with their needs. 
• Securing the services of the Registrant's attending Physician when indicated 
• Direct and Supervise services, function and staff in the following departments: Social Services, Dietary, Recreation and Rehab. 
• Responsible to submit a monthly report to the Administrator 
• In conjunction with Medical Director, oversee and coordinate Ancillary Services 
• Responsible to develop, complete and present a yearly program evaluation 
• Responsible for Infection Control policies and Procedures 
• Responsible for Quality Assurance 
• Responsible for Staff Development and in-services 
• Responsible for all other duties deemed necessary by the Director of ADHC
• Must be able to commute between our two communities.
