Provide patient-centered care utilizing nursing knowledge.

  • Meet patient needs while rooming and assessing the patient.
  • Documents care provided in Epic
  • Nursing care provided utilizing St Elizabeth values, Joint commission regulations and Medical Home Model attributes.
  • Maintain licensure, skills and competency
  • Work collaboratively with interdisciplinary team members to plan and deliver care.
  • Serve as patient advocate.
  • Responsible for teaching, mentoring, training, and supervising licensed and unlicensed personnel, based on their training and education.

Comprehensive Care

  • Provide Nursing within the team to meet patient needs based on age, health and medical conditions.
  • Provide document in epic while rooming a patient for physician.
  • Assessment of patient to include poct testing, vaccines, medications, along with allergy review , smoking and medication reconciliation.

Coordinate Care

  • Provide open and clear communication with patients and team members to meet patient needs thru a team based approach.
  • Stays current with Joint commission regulations and provide guidance and support to team members to maintain compliance.
  • Assist, support and teach new Physicians during their time in the Residency Program.
  • Function as staff resource for team members
  • Function as staff contact person in the absence of Clinical Coordinator
  • Contacts appropriate Management staff as need arises.

Patient Safety

  • Demonstrate proficiency in doing nursing tasks with accountability.
  • Stays current with Patient safety and infection control guidelines.
  • Provide care within regulations and guidelines.
  • Provide POCT testing per regulations
  • Provide teaching of immunizations
  • Administer vaccines and medications per regulations

Quality/Customer service

  • Provide population based care using hospital St Elizabeth values.
  • Provide nursing clinical support tools for shared decision making with patients and families.
  • Involvement in performance improvement efforts

Financial Responsibility

  • Support budget by appropriately utilizing resources and using supplies in a cost effective manner.
  • Maintain patient flow, be on time and productive.
  • Stay current with VFC and regular insurance vaccines and use correct stock.

Maintain Clinical area and patient rooms

  • Opening and closing duties
  • Maintain room assignments
  • Assist in maintaining stock needed for clinical area, patient rooms and lab

Performs other duties as assigned.

Qualifications

Education, Credentials, Licenses:

Graduate from accredited School of Nursing, licensed to practice nursing in the state where work is being performed.

BSN, Clinical certifications and specialized courses

Specialized Knowledge:

Demonstrate knowledge of nursing theory, clinical practice standards and policies. Competent to independently perform safe nursing skills and procedures.

Based on previous work experience demonstrates proficiency in the performance of nursing practice. Knowledge of Joint commission Regulations and Medical Home Model.

Kind and Length of Experience:

Prior experience as an RN. Able to critically think and problem solve in routine and emergency situations.
