Job Description

Isle Health and Rehabilitation located in beautiful Fleming Island, FL has an opportunity for an RN/LPN (Registed Nurse/Licensed Nurse) MDS Coordinator to oversee and coordinate the development of the member assessment (MDS) in accordance with current Federal and State regulations. The ideal candidate will be team oriented, resident focused and self-motivated to ensure compliance is maintained at the facility as it relates to member assessments.

 

·         Must be an active Registered Nurse (RN) or Licensed Practical Nurse (LPN) in good standing licensed by the state where our facility is located.

·         Must have at least 2 years’ experience as a Nurse.

·         Must have knowledge in PPS assessment as well as OBRA compliance.

·         Demonstrates clinical assessment skills at the level necessary to meet the job requirements.

·         Ability to make independent decisions and follow through with limited supervision. Knowledgeable and understanding of state and federal regulations and guidelines governing the practices of the facility.

·         Effective interpersonal skills and the ability to work cooperatively with an interdisciplinary care team.

·         Able to relate positively, favorably, and cooperatively with others, including, members, family members, associates and personnel of outside agencies and organizations.

·         Utilizes the current RAI Manual as a resource during the assessment coding process.

·         Schedules the Assessment Reference Dates (ARDs) for payment assessments in a manner that accurately captures each resident’s clinical characteristics for payment classification purposes.

·         Schedules the ARDs for all OBRA (Omnibus Budget Reconciliation Act) assessments daily, monthly, and as needed.

·         Provides a schedule of ARDs and assessment types weekly and as needed to the interdisciplinary team (IDT) to facilitate the timely completion of MDS sections and CAAs (Care Area Assessments) by each discipline.

·         Transmits assessments in accordance with current regulations.

·         Facilitates the completion and updating of resident care plans with the IDT to reflect each resident’s current needs.

·         Performs administrative duties as assigned, including but not limited to QAPI activities and participation in department or committee meetings.
