Welcome to St. Francis Center for Rehabilitation & Healthcare! Our 273-bed facility, provides a warm and caring environment for short-term rehab patients, and long- term residents including memory care. Our team of seasoned staff work together to offer excellent service and care to each resident. Our nurses and Certified Nursing Assistants deliver excellent nursing care while adhering to the Pennsylvania Department of Health regulations and Federal Health Care Administration.

Registered Nurse (RN) and Licensed Practical Nurse (LPN) Responsibilities:

Provides quality nursing care to residents to maintain or attain the highest practical level of functioning

Assesses residents for any decline of condition, signs of change in condition, weight loss, decline in ambulation, change in ADLs, development of skin breakdown, etc.

Notifies appropriate staff per facility protocol of the need for evaluation and informs staff on any changes in assessment when appropriate

Administers medications and treatments to residents which includes; monitoring administration of intravenous fluids and placing orders for medication

Complete resident incident reports and complete follow-up investigation forms

Maintain accountability for controlled substances by reviewing inventory medications with another licensed nurse and notifying supervisors of any discrepancies

Supervise the CNAs in regards to work procedures and expediting work flow

Performs other related job duties as required

CNA Responsibilities:

Care for residents in a manner conducive to their safety and comfort, under the direction of a licensed nurse

Assist RNs and LPNs as needed

Perform ADLs, such as: bathing, dressing, serving meals, feeding, ambulating, turning and positioning

Maintain clean living conditions for residents and report on resident status or family concerns to a licensed nurse

Ensure that all residents receive the highest quality of service in a caring and compassionate environment, which recognizes the individual's needs and rights

Performs other related job duties as required

Registered Nurse (RN) and Licensed Practical Nurse (LPN) Qualifications:

Graduate of an approved school of nursing is required

Must have a current Pennsylvania RN or LPN license

CNA Qualifications:

Must have an active certification as a Certified Nursing Assistant (CNA) in the State of Pennsylvania
