Palatka Center for Rehabilitation and Healing is now hiring an experienced MDS Care Plan Coordinator who is committed to providing the best life and care for our residents. Under the: direction of the Director of Nursing, the MDS Care Plan Coordinator is responsible for implementing the federally mandated assessment tool to identify needs / concerns of the resident and to coordinate a plan of rare to assist the resident toward optimal wellness based on the highest practicable level of functioning.

We are located at 110 Kay Larkin Dr, Palatka, FL 32177.

MDS CARE PLAN COORDINATOR QUALIFICATIONS:

• Must possess current and valid state Registered Nurse ( RN ) license
• Must be able to relate positively and favorably to residents, families and outside agencies and to work cooperatively with other associates
