Job Description

A Binghamton area skilled nursing facility is currently searching for a full-time RN MDS Coordinator.  

The RN MDS Coordinator is responsible for organizing, directing and coordinating MDS assessments and PRI’s in compliance with Federal and State standards, guidelines and regulations. The MDS Coordinator will also be responsible for ensuring the highest degree of quality care is maintained by acting as a resource for the staff.

Primary Responsibilities 

  • Completing accurate assessments, MDS & care plans as assigned;
  • Arranging schedule of all assessments required in coordination with resident care plan meetings and CMI schedule;
  • Working knowledge of new PDPM regulations;
  • Knowledge of CMI with understanding how to maximize CMI score;
  • Initiating care plans and supporting activities as assigned;
  • Creating and distributing monthly care plan calendars in a timely fashion;
  • Maintaining & updating all care plans and assessments as required;
  • Monitoring & auditing clinical records, ensuring accuracy & timeliness;
  • Informing DON of persistent issues related to non-compliant documentation;
  • Protecting the confidentiality of Resident & Facility information at all times;
  • Ensure all strategies comply with Medicare requirements and ethical standards;
  • Must be a team player and have excellent communication skills with all department;
  • Coordinates with Nursing Educator and Nursing Administration to ensure appropriate teaching of nursing and interdisciplinary staff;
  • Participates in MDS training of staff as required.

Skill and experience requirements:

  • Must be a Registered Nurse (RN) or completed a registered nursing program
  • Must have MDS Coordinator experience
  • Able to read, write and understand English
  • Need to be dependable, adaptable to change and able to work in a fast-paced environment
