Job Description

New Vanderbilt Rehabilitation and Care Center is looking for a Nursing Care Coordinator to provide day-to-day management and oversight of a skilled nursing unit. 

Duties and Responsibilities

• Implementing, evaluating, and directing the day- to-day functions of a skilled nursing unit.
• Provide written and oral reports or recommendations to the Director of Nursing Services, as necessary or required concerning the operation of the unit.
• Participate in the facility's quality assurance program.
• Provide leadership and direction to the Certified Nursing Assistants and Nurses assigned to the unit.
• Ensure that routine nursing care is provided to meet the daily nursing care needs of each resident.
• Review nursing notes to ensure that they are informative and descriptive of the nursing care being provided, they meet the needs of the resident's Care Plan and that care provided is in accordance with the resident's wishes and needs.
• Conduct daily rounds to observe residents and to determine if their nursing need are being met.
• Providie Direct Nursing Care as necessary.
• Report medication errors to the Director of Nursing.
• Review and revise care plans and assessments as necessary but at least quarterly.

Qualifications

• Must possess a valid unencumbered NY RN license.
• Bachelor Degree in Nursing from an accredited college or university, preferred.
• 2 years of supervisory experience in a hospital or skilled nursing facility.
