About us

Collar City Nursing and Rehabilitation Center is a 120 bed skilled nursing facility, conveniently located in the beautiful upstate New York Capital Region. The Diamond is centrally located, a short distance from Albany, Rensselaer, Clifton Park and Saratoga Springs. The facility provides inpatient skilled nursing and rehabilitation, wound care therapy, and intravenous (IV) services.

Responsibilities

Candidate must be a Registered Nurse in good standing with 2-5 years' experience in LTC, supervisory experience preferred.

· Duties include, but are not limited to, managing resident care.

· Supervision of CNAs and LPN

· Candidate will work closely with Director of Nursing to ensure Department of Health compliance with regulations, as well as adhering to company policies and procedures.

· Basic computer knowledge, MDS proficiency and knowledge of care plan process is required.
