Responsibilities Include:

•  Assessing all new admissions skin integrity
•  Document stage, size of pressure ulcers
•  Assess wounds and document number of staples/sutures
•  Ensure orders for treatments are in one place -
• Ensure all preventive measures are in place (ie) heel bootie, air mattress etc.
• Ensure CCP reflects pressure ulcer (CCP for each site) and treatment plan for each site
• Ensure CNA tasks reflect pressure ulcer, and turning and positioning, toileting schedule , preventative devices etc.
• Coordinates weekly wound rounds with wound doctor and unit care coordinator.  Makes sure wound documentation is completed
• Ensure weekly flow sheet is done - Ensure Braden scale is updated with change in pressure ulcer - Notify family of change and document - Email flow sheets to departments heads
