Position Summary: The Swing bed Activities and Clinical Coordinator is responsible for planning and directing daily Swing bed activities, medical staff, ancillary departments, patients, and families. This position provides for the delivery of care to a specific group of patients and evaluates the quality of that care in a systematic fashion, in order to improve care. Care will be delivered within the framework of hospital philosophy, approved budget, and in concert with other departments and services within and outside of the hospital. This position must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served. Responsibilities DUTIES, ACCOUNTABILITIES AND RESPONSIBILITIES: *Identifies patient needs and develops an action plan for development and implementation by assessing, planning, implementing, and evaluating services to meet the needs of the customer. *Demonstrates excellence in verbal and written communication. *Facilitates integration of patient care services throughout the organization by coordinating all services to provide and implement appropriate nursing and clinical interventions. *Assumes accountability for patient care delivery in the department. * Responsible for ordering supplies * Plans and directs an activity program that meets needs of various patient function levels. * Assists Med/Surg Director with daily operations * Ensures patient receive follow-up after discharge. * Helps with staff education including annual and ongoing in-services. Job Requirements (Qualifications/Requirements): *Graduation from an approved school of nursing and licensed as a Registered Nurse (current licensure) in the State of Texas. Bachelor of Science in Nursing preferred. *Experience: At least two years recent experience in the nursing specialty preferred. *BLS Required *ACLS Required *PALS Required *Successful completion of Texas State approved Activity Director’s Course PHYSICAL REQUIREMENTS: * Must use written, verbal, non-verbal, and auditory sources of information. * Must use oral communication to perform work. * Must possess digital dexterity. * Must make general decisions in the absence of specific directions. * Must be able to lift 50 pounds. * May be required to lift in excess of 50 pounds with assistance. ENVIRONMENTAL CONDITIONS: *This is an OSHA category I position. Body substance isolation must be practiced.
