What You Will Do:

1. Responsible for quarterly assessments, the MDS/Rap modules, and input into the care plan process.

2. Oversees all admissions to the facility for JCAHO standards. Ensures all paperwork is properly executed (i.e., Admission’s Comprehensive Assessment, MD Orders, lab work, etc.).

3. Ensures weekly wound documentation is appropriately noted.

4. Evaluates, recommends, and assists with resident treatments.

5. Assumes responsibilities for providing all CNA’s with their daily assignments.

6. Responsible for maintaining all resident profiles.

What You Will Need:

1 Ability to read, write and comprehend English and to follow oral and written instructions.

2. Interest in the needs of the aged and the chronically ill with the ability to work with the elderly.

3. Ability to work hours as scheduled based on the requirements of the position/assignments.

4. Registered Nurse licensed in the State of New York. Minimum of one year of education or experience in geriatric nursing or rehabilitation preferred.

5. CPR certified.
