REGISTERED NURSE HOME HEALTH-INFUSION THERAPY

LOCATION:  NORTH SUBURBS

HOURS:  DAYS

This position is also eligible for a $10,000.00 sign on bonus with 50% payable upon hire and 50% payable at 1year!  You must be employed in the exact position at time of payout.  Internal team members are not eligible for this sign on bonus at this time.

Position Requirements:
Graduate from an accredited school of nursing
Minimum 2 years Medical/Surgical experience
Critical care experience a plus

Good communication and interpersonal skills
Good documentation skills
Good problem solving skills
Ability to determine appropriate frequency of visits

CPR
Current licensure in the State of Illinois as a registered nurse
CRNI preferred

Extensive travelling within geographical boundaries in all extremes of weather.
Ability to transport patients is required of this position.
Exposed to hazardous materials with respect to patient care.
Exposed to widely varying social and environmental conditions in the patient's neighborhoods and homes.
Should have some means of insured transportation.
Involves high travel and large territory coverage.
On~call responsibilities may be frequent.


If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case~by~case basis.

Purpose:
The purpose of this position is to help achieve the vision of Advocate Home Health Services by performing the duties of this position in a cost effective, results oriented and customer focused manner. This position is a critical component of the Infusion Nursing team and as such meeting individual performance expectations and contribution to team performance goals is required.

Provide quality nursing care to home infusion therapy patients in accordance with physician's orders, recognized nursing techniques, Advocate Home Health Services policies and procedures and standards of care. Maintains a holistic perspective of the patient/family according to the patient's physical, social, spiritual and psychological needs.


Accountabilities:
Responsibility for patient outcomes through application of the nursing process involving optimal level of self~care for patient and optimal quality of life for patient, family, significant others.
Provision of positive role model for other health care team members.
Maintenance of professional skills and growth.
Generation and collection of patient care revenues directly related to productivity, determination of appropriate frequencies, adherence to frequencies, quality of documentation including appropriateness and timeliness.
Minimization of patient/family knowledge deficits regarding disease process, patient/care, medications, infection control and specific procedures utilizing appropriate teaching processes as well as sharing professional knowledge base with other health team members.
Responsibility for human relations with patients, families, referral sources, physicians, community services and vendors through the use of appropriate, effective, facilitative interpersonal communication skills.
Achievement of agency, department and personal goals and objectives.
