Summit Home Care is a family-owned and operated home care agency.  We are currently looking for compassionate and reliable RNs & LPNs to join our team of dedicated nursing professionals. If you are an exceptional nurse, both registered and licensed look no further and apply today.

We are a full-service PDN agency that services both the pediatric and geriatric populations.  We have an immediate need for LPNs to fill shifts, nights, days, and weekend hours.  This opportunity awaits motivated and ambitious professionals looking to work under the direction of our Nurse Case-Managers.

Responsibilities:

·         Follow a plan of care designed by our Nurse Manager

• Perform Medication Administration and Treatments per POC
• Monitor clients' conditions and report accordingly
• Accurate documentation in accordance with the provided orders

Requirements: 

• Current LPN license in the state
• 2+ years' nursing experience
• Up to date first aid and CPR certification
• Willingness to work evenings and weekends
• Exceptional staff training and customer service skills
• Compassionate and friendly demeanor
• Required Medicals
