Restorative Nurse - Restorative Certified Preferred !!

Job Summary: Responsible for the directing of Restorative Services using person centered. Coordinates and supervises restorative programs with in policies and procedures of the facility. Maintains efficient and effective department operation while requiring compliance with all state, federal and local regulatory laws, standards and protocols.

DUTIES AND RESPONSIBILITIES:

Responsible for direction of Restorative Nursing Services staff. Supports and empowers staff to embrace the mission and value statements of the organization.

Coordinates schedule and programming with therapist.

Educate nursing staff on therapy programs and restorative programs using a person centered care approach. Explain to staff how to assist with goal achievement.

Oversee scheduling of patients. Ensures that patients are appropriately evaluated and treated. Reviews medical records upon admission for therapy orders to evaluate and treatment then refer to therapy.

Review medical record for documentation supporting the need for skilled therapy. Continue to monitor for compliance.

Evaluate and complete all required restorative assessments upon resident admission, quarterly, annually and as needed.

Restorative assessments will include ADL bathing, dressing, eating, mobility, strengthening, bowel and bladder, PROM and Active ROM.

Direct and supervise staff to carry out restorative programs. Give additional instruction to staff as needed.

Complete MDS, CAA and care plan process. Attend care plan conferences as scheduled and be prepared to discuss residents status related to ADL function/programming, rehab/therapy programming falls and restraint usage.

Ensure Kardex in the EMR is current to direct the care needs of all residents to staff.

Attend Medicare and Medicaid RUG meetings and be prepared to discuss resident’s status and potential to maximize reimbursement. Have available information for 7 day look back period.

Able to assess patient pain and if it is interfering with optimal level of function or participation in rehabilitation or restorative programs; makes appropriate physician contact for intervention.

Coordinate and manage the POC aspect of the EMR including the education of the nursing staff, implementation of the resident tasks, and completion of the POC documentation.

In coordination with therapy assess residents for proper type and fit of wheelchairs, splints and other adaptive or restorative equipment.

Knowledge of disease processes and related causative factors. Supports and maintains a culture of safety and quality.

Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning, family/ resident representative is included in teaching as appropriate.

Demonstrates knowledge and proficiency of restraints: physical, chemical and seclusion. Able to explain restraint policy, including assessment, evaluation, documentation, orders and regulations.

Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families. Functions as liaison between Director of nurses, therapists, patients, physicians and other healthcare providers.

Knowledge of infection prevention and control, sterile technique and related impact on restorative services. Knowledgeable of facility antimicrobial and CAUTI programs.

Manages and operates equipment safely and correctly. Maintains department cleanliness and safety. Complies with ongoing departmental inspections of equipment and physical space, makes recommendations regarding equipment utilization and needs.

Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.

Communicates appropriately and clearly to physicians, staff and administrative team. Uses SBAR as indicated.

Coordinates and directs patient care to ensure patients’ needs are met and facility policy is followed.

Understands the stages of dementia and techniques for communication with cognitively impaired residents.

Knowledgeable of the national patient safety goals.

Maintains a good working relationship both within the department and with other departments.

Consults other departments as appropriate to collaborate in patient care and performance improvement activities.

Maintains performance improvement activities for department.

Participates in new hire orientation and in-service training, participates in guidance and educational programs.

Documentation meets current standards and policies.

Administers medication following the 7 medication rights reducing the potential for medication errors.

Trains staff utilizing on-the-job training for C.N.A.’s, nursing and other staff.

Studies current trends and developments in restorative medicine through current literature and professional society.

Maintains standards of professional society procedures and ethical behavior.

Maintain adequate materials, records and quality controls in accordance with hospital standards.

Demonstrates an ability to be flexible, organized and function under stressful situations.

Completes other duties as assigned.

Professional Requirements:

Maintains regulatory requirements, including all federal, state, local regulations and accrediting organization standards.

Ensures compliance with policies and procedures regarding department operations, fire safety, emergency management and infection prevention and control.

Complies with all organizational policies regarding ethical business practices.

Communicates the mission, ethics and goals of the hospital, as well as the focus statement of the department.

Regulatory Requirements:

· Completion of a recognized Restorative Nursing Program or     willingness to complete.

· Current Nursing license in the state of Illinois.

· At least two (2) years previous experience in nursing.

· Current BCLS certification.

Language & Skills:

· Able to communicate effectively in English, both verbally and in writing.

· Additional languages preferred.

· Strong written and verbal skills.

· Basic computer knowledge.
