Job Summary and Qualifications

As a Social Worker LCSW you will provide assessments, diagnosis mental health issues, counsel and referral for treatment and/or higher level of care to recipients and donors throughout the transplant process.

What you will do in this role: 

  •  Provides mental health screenings in clinics, hospitals, and other settings to patients have been medically referred. Reviews medical and social case histories as needed. 
  • Diagnoses mental health related issues including but not limited to depression, anxiety, substance abuse concerns. Works closely with consulting psychiatrist and clinical psychologist on challenging cases. 
  • Develops an individual plan of care for patients to include treatment of mental health related issues, supportive counseling, crisis intervention, psycho-education, education related to accessing healthcare services at the appropriate level of care; preventative education, and community based resources. 
  • Documents professional recommendations, care coordination interventions, and treatment plans timely and accurately to communicate to all members of the health care team. Implements and adheres to policies to ensure patient confidentiality. 
  • Facilitates referrals to other mental health professionals within program and other community based agencies with regard to clients and/or patient care.
  • Participates with the interdisciplinary team to ensure all psychological/psychosocial needs are addressed and offers recommendation for transplant/donation candidacy.

What qualifications you will need:

  • Education: Masters in Social Worker from an accredited school
  • Experience: Two years as an LCSW or 1 year LCSW with transplant experience
  • Must be licensed as a Clinical Social Worker with the Texas State Board of Social Work Examiners.
