Are you a licensed social worker looking for fulfillment working in hospice care? Set on the Mississippi River in western Wisconsin, La Crosse is a beautiful waterside city with a variety of fun things to see and do. You must have a Wisconsin license and Minnesota (or be in the process of obtaining your license in MN). myPTsolutions is assisting its client to hire a licensed social worker for 13 weeks. You will be fulfilled serving a mix of all patients in an inpatient care including ortho and neuro included, heavy TBI, Stroke, MVA. You will assist in all units within the hospital, and where assistance is needed. The shift is 8+ hour days, 7:30-4 and 8-4:30. There is the potential to go over 40 hours per week.

Compensation of the Social Worker:
Pay: $51 - $56 / hour based on experience, benefit level and traveler status.

Responsibilities of the Social Worker:
Provide assistance to aid patients with community resources, financial, or placement into an alternative setting.
Responsible for integrating, facilitating, and coordinating patient care
Excellent communication and interpersonal skills.
Provides direct clinical services including, individual therapy, family therapy, group therapy, and counseling as appropriate
Provide case coordination and therapeutic education services.
Provide excellent patient care and compassion.

Requirements of the Social Worker:
Required Certifications: Bachelor's Degree in Social Work or related field.
Must have at least 1+ year experience as a social worker
Inpatient hospital and discharge planning experience required
Ortho/Neuro experience is a plus
Hospice experience required
Charting Experience: EPIC
Wisconsin and Minnesota SW License
ACLS, BLS, Covid Vaccine
