Shift Details: Shift 11am-7:30pm

Valley View Manor is looking for an energetic and compassionate Wound/Admissions Nurse (RN) for our 82-bed facility located in Norwich, NY.

The Wound/Admissions Nurse is a Registered Nurse (RN) that provides direct care, to coordinate care related to all residents upon admissions. The RN will also manage all high-risk skin injuries and provide recommendations for residents who are at risk for skin breakdown.

Responsibilities:

Completing the required assessments and documentation for the effective management of new residents including: therapeutic nursing interventions, resident and family education, goal setting, critical thinking, and care planning.

Consult to recommend best practice for complex wounds, ostomies, and preservation of skin facility- wide.

Data collection and reporting quality outcomes for quality assurance and performance improvement.

Effectively communicate with the interdisciplinary team.

Qualifications (Degree, Certification, Experience):

Must be a graduate of an accredited school of nursing, college or university for Registered Professional

Nurses.

Must possess a current NYS license and registration as an RN.

A minimum of 1-2 years of recent acute/long-term care management preferred.

Competence in head-to-toe assessments

Competence and experience with identifying, staging, and managing wounds

Strong written and oral communication skills.

Supports facility management and staff by facilitating change in a positive manner.

Ensures a safe and secure environment for residents, staff, and visitors.

Takes responsibility for own education and stays clinically current.

Obtain and implement physician orders. Ensures documentation is current and accurate.

Provide education and instruction to resident/patients, families and care givers as required.
